Below are Patient Forms which you may download to your computer prior to your visit, complete, and bring with you on the day of your appointment. This will help save you time.

We reserve a significant amount of time for you, and our staff invests a great deal of time in scheduling and registering you for your appointment. Please understand that untimely cancellations in our schedule creates problems for our office plus deprives the opportunity for another patient to be helped at that time. Most importantly, it jeopardizes our ability to ensure that you receive the dental care you need in a timely manner.

We do understand that in some situations it can be unavoidable to change your appointment. Respectfully, we request that you give our office 24 hour notice if you need to cancel your appointment.

We accept:
-Most major credit cards (w/ symbols–MasterCard, VISA, Discover)
-Care Credit (as a link to Care Credit), paragraph description of Care Credit and “Please click on the link for more information and the application process.” (we’ll need to get info to you on Care Credit as well)

Dental Insurance
While dental insurance is not required for treatment in our office, we do accept many dental insurance plans. We will gladly file your dental claim with your insurance company to help you receive the maximum benefit your policy allows.

**If your dental insurance is not listed, we will still process your claim with your insurance company. Please speak with our office staff to further help you in this situation.
